In the event of a legal name change, different from preferred alias, users are able to submit a request to Information Technology Services for permanently changing a UofL Username (e.g. fmlast01).
Step 1
Navigate to https://louisville.edu/its/tech-support/forms/forms
Step 2
Expand UofL Account (UserID) Name Change Request
Step 3
Select Request
Step 4
Login to ULServices with your ULink username (e.g. fmlast01) and password
Step 5
Select Request for UserID change based on a Name Change
Step 6
Complete and Send your request to ITS
